- Character can permanently lose the ability to jump (solution: quit mission).
- Character can permanently lose the ability to move (solution: quit mission).
- Players and AI characters can lose the ability to move and start moving slowly while staying stuck into the ground, sliding into one direction until map obstacles stop them, or get killed by map hazard elements. If they try to move, animations still play, but no effect.
- If a character pauses while reloading, the reload animations will still continue while paused. The clip on the gun model will also be invisible until the gun is switched or reloaded again if the pause occurs in the animation phase of when the clip is not included in the gun. This is also a bug in other SFH games too.
- When reloading the LAW Giver the receiver cover that you opened changes its color into standard weapon colors and reverts back when it is shut back down on the gun.
- The magazine of the Righteous Bizon when equipped on a hero is the normal color instead of the usual golden.
- Using a blade/Deadly Shurikens make the user's body glowing up, similar to firing a gun. In the case of Deadly Shurikens, it even drops the casings after fire.
- Respawn timer can restart while counting, or not start at all. It usually fixes itself after a long period (ca.10 seconds - if not, just quit and retry).
- If your Ninja or Mercenary ally activates the Deadly Shurikens or Betsy killstreak, when the AI runs out of shuriken/ammunition they will get stuck with no shuriken and as a result, they can't kill enemies (if they don't get killed, you can betray them and fire rockets/grenades/elemental effect at them to kill them, it's the only way out).
- Swinging Katana and Hattori Hanzo during "I'm the Juggernaut" will disable the shield.
- Sometimes when an Elite activates Satelite Beam killstreak, a character will stay frozen, even if the ice disappears, and will not react to anything even if the freezing effect goes away. This usually gets fixed after a long period (ca. 30 seconds-or just start a new game).
- Sometimes the character can suddenly die on certain spots in some maps, such as dying on stairs in the "Volcano" map.
- Playing on "Forest" map will periodically freeze your framing (a.k.a drop your fps to 0) for an extremely short window.
- On the Factory map, you can get stuck on the truck on the far right.
- In Junkyard, standing near the giant magnetic beam for an amount of time will kill you even if you aren't affected by the pulling effect of the beam.
- Enemies randomly commit environmental suicide through cliffs or hazards, like using the magnetic beam in 'Junkyard' map and randomly falling through lethal holes. This is also very common in Strike Force Heroes 1 and 2 (especially in the clone factory in SFH2 and the GlobeX base in SFH1).
- Medics with the Trait 'Loving Spirit' actually make the ally respawn times even longer rather than making it 3x shorter.
- When you have a custom weapon and die, your weapon color turns from gold to the normal weapon color.
- Fiesta mode does not work.
- The trait Vital Bomb does not work.
- The Scavenger mod/skill will refill your spare (in-clip if you are using a single-clip MG or have the Drum Mag) ammo instead of giving you back 10%, like it says.
- Taking ammo from the ammo pedestal while reloading has a chance to reset your reloading progress.
- Sometimes, when choosing heroes in the lobby, you will find that your heroes lose their primary weapon. Weapons can also be held incorrectly (i.e: holding a blade in AR stance) occasionally. It can mostly and easily be seen when a new hero is recruited.
- Rusty (Knight) on AI controls prioritizes the secondary blades if the primary blade does not have longer range.
- In Gun Game/Team Gun Game mode, sometimes the equipped pistols on yourself and other AIs swapping their own models into another pistol models several times in a second before returns to normal state.
- In CTF, sometimes the allied AI flag carrier is freezed in place without delivering them to the base and enemies stop spawning, effectively halting the game.
- Leveling the AIs too high using cheats causes the AI behaviors to shift; the AIs will spin around firing their weapons when facing an enemy. An example is found in side mission 8, The Hitman.
- Using a Ninja with the "Chameleon" perk on the "Watch Your Back" mission will remove the permanent invisibility and instead use the "Chameleon" perk.
- In some versions of the game pressing the start mission button is impossible and instead selects one of the heroes.
- Burst Module applies for both primaries and secondaries, and also works on blades.
- The GUI will display the secondary weapon's name as the currently equipped primary weapon if that hero is controlled by AI, had emptied their primary weapon clips, is currently using the secondary and is chosen as your hero after you die. The names will be corrected as soon as you switch back to the primary weapon.
- Using a weapon-modifying killstreak while carrying a flag makes the suitcase behind you disappears (aesthetics only).
